Declaration of consent

If you are under 18, a parent or legal guardian must be present in person at the appointment.

The following contract is concluded between My Story Frankfurt, Inhaber Lars Reinschild, Hanauer Landstr. 64, 60314 Frankfurt am Main (the "studio") and the person named above.

• I understand that there is a risk and that a form of bodily harm and body modification will be carried out.
• I understand that the tattoo may differ from the original design.
• The tattooing process was explained to me in detail.
• The aftercare for the tattoo was described to me thoroughly and adequately, and I have understood it.
• Before tattooing begins, I check the design, placement and spelling and approve them.
• I know that if the session has to be stopped because the pain is unbearable, it must be paid in full and a new appointment must be booked with a surcharge.
• The session will be stopped, and must still be paid in full, if anything on my part prevents the artist from working undisturbed.
• We advise against tattoos on the hands, fingers, feet, lips and head. Touch-ups in these areas are therefore always charged for.
• On all other areas, a touch-up is free of charge within three months of the original appointment if one is needed. The tattoo artist or studio decides, based on professional judgement, whether it is needed.
• With cover-up tattoos, the old tattoo may still be visible. Everyone's skin is different, so it is impossible to say in advance how the new tattoo will cover the old one. I confirm that I understand this.
• The studio is not responsible for consequences resulting from my failure to follow the aftercare instructions (for example improper aftercare, unsuitable ointment, lack of sun protection). The studio's liability for intent and gross negligence and for injury to life, body or health remains unaffected.
• By concluding this contract, I agree to the terms and conditions.
